History
OHR and widespread mammary tumors in September. Tumors ranging in size
from mm to cm have developed along incisions since then.
Histologic description
Several samples all with disorganized epithelial proliferation forming densely
cellular solid lobules, occasional cysts and in one are acinar structures. The
mitotic rate is high and HPF with 5 or more spindles are not uncommon.
Diagnosis
Mammary carcinoma (solid)
Comment
The neoplastic cells show significant atypia (high N:C ratio and a high mitotic
rate). Despite this, the growth pattern of the mass suggest a malignant tumor
which considering the site, is not especially aggressive (the tumor is expansile
rather than clearly infiltrative). Based on the histologic evaluation, complete
excision of this type of tumor would be expected to be curative. But, the
macroscopic description is of concern. Further, although lymphatic invasion was
not seen, the validity of this observation is questionable, given the very small
amount of the tissue surrounding the tumors available for examination. In
summary, the tumor is of concern in this case. Only parts of the masses are
available for examination and no comment can be made regarding the margins of
the excision.
